Reentry, stabilization, and leadership development for justice-impacted and at-risk youth ages 13-18. 

Referral Criteria

Who Can Be Referred

  • Youth ages 13-18 who are justice-impacted, at-risk, recently released, or preparing for release.
  • Youth currently involved with probation, juvenile court, school discipline systems, or child welfare systems.
  • Youth returning home, transistioning to community placement, or navigateing early reentry.
  • Youth who demonstrate willingness to engage in mentorship, skill-building, and supportive services.

Who Can Refer

  • Probation Officers
  • Juvenile Court or DJJ staff
  • School Counselors and Social Workers
  • Case Managers
  • Community-Based Organizations
  • Parents or Legal Guardians (with appropriate documentation)

All referrals are reviewed on a rolling basis by the Reviving Leaders Collective team. 

What is Rise & Reclaim?

Rise & Reclaim is Reviving Leaders Collective’s flagship reentry and stabilization program designed to support justice-impacted and at-risk youth ages 13-18 as they transition home, prepare for release, or navigate early reentry. 

Delivered through structured 8-week cohorts, Rise & Reclaim provides mentorship, life skills development, workforce readiness, and community-based stabilization support tailored to each youth’s stage of transition.

Youth are referred by trusted partners and assessed based on readiness, need, and stage of transition.

Youth participate in mentorship, life skills, leadership development, and workforce readiness supports. 

Youth are connected to ongoing supports and next-step opportunities for school, training, and community engagement. 

Program Components

  • Builder’s Table- Leadership development and workforce readiness embedded into Rise & Reclaim cohorts.
  • Circle 7- Peer support groups reinforcing healing, accountability, and connection. 
  • Justice Bridge (Supportive Service)- Donor-supported stabilization and bond assistance offered separately as needed.
  • Legacy & Liberation- Alumni connection and continued support beyond program completion.

  1. How long is the Rise & Reclaim Program? Each cohort runs for approximately 8 weeks, with ongoing support available based on participant needs.
  2. Is the program only for youth recently released? No. Rise & Reclaim supports youth at various stages of reentry, including pre-release planning, immediate transition, and early community reintegration.
  3. Is there a cost to participate? No. All services are provided at no cost to referred youth and families. 
  4. How soon will I hear back after submitting a referral? Referrals are reviewed soon a rolling basis. Our team responds within 24-48 hours.
  5. Does a referral guarantee acceptance? No. Acceptance is based on eligibility, capacity, and program fit.
